Closing the Research Gap: Enhancing Paediatric Care for Chronic Bowel Conditions

Having reviewed the literature around transanal irrigation for paediatric care, I was surprised by the limited amount of research in the last 5 years. Many papers rightly call for more research in these areas, but we must acknowledge that expanding the evidence base is not always straightforward. In the current healthcare climate, where funding is often stretched and resources are prioritised toward other areas, conducting new, large-scale studies is no simple task. 

Having said that there are specific papers which can help guide clinical practice. One particular paper is ‘Consensus review of best practice of transanal irrigation in children’. Although this is now 8 years old it remains highly relevant, and provides recommendations on indications, patient selection, patient and family training, troubleshooting and practical aspects of transanal irrigation. With a conclusion suggesting that thorough training and careful follow up are key to transanal irrigation success.  

A more recent, 2024, paper ‘Transanal irrigation in children with functional constipation: A systematic review and metaanalysis’ was published.  The authors concluded that overall, only five studies could be included in the analysis, plus these were not of robust methodological quality and used different definitions of treatment success. Therefore, the evidence has a propensity to be insufficient.  Thus, calling for welldesigned prospective trials to evaluate this treatment option in children with refractory functional constipation. 

I had always believed that the body of research on this topic was more extensive than what I was able to find. The children affected by chronic bowel issues warrant the most informed care possible. Stronger research would go a long way in making that a reality. While the current studies are helpful, a continued push for expanded data would undoubtedly strengthen the foundation of paediatric care in this area.
